We thank you for your interest in Pro-Fit Physical Therapy & Sports Medicine. Our goal is to provide every physical therapy patient with superior customer service and stellar care.

When calling to schedule your appointment please have your insurance card out. Be prepared to give a member of our office staff your basic information, including your insurance carrier, member ID number, claims address and telephone number listed on your card.

You can take an active role in your rehabilitation by:

  1. Making every physical therapy appointment
  2. Completing the recommended stretches and exercises daily
  3. Being honest about your progression with therapy
  4. Always asking questions

We value our time with you. Therefore, it is imperative that you contact our office 24 hours prior to your appointment time, should you need to cancel or reschedule your appointment. This ensures that you are not charged a $30.00 no-show fee.

To be seen by our Physical Therapist we require all Personal Injury, Workers Comp, Auto Accident and Sports Injury patients obtain a referral from their (PCP) Primary Care Physician prior to scheduling.

Note: If you do not have a Primary Care Physician we will be happy to refer you to one.

We accept virtually all insurance plans. However, you maybe responsible for copay’s, deductibles and co-insurances based on the specifications listed on your plan for Physical Therapy.

The following insurances are contracted with specific Physical Therapy Offices:
